New Patients
If you are interested in joining Mungret Medical Centre please complete and send us a New Patient Form.
New patients, Medical Card, Doctor Visit Card, Under 8, Over 70, and Private patients welcome.
The New Patient Form can be filled out and submitted online, and can also be downloaded as an editable PDF or collected in paper form from MMC.
The New Patient Form allows us to set you up on our electronic health record system, please ensure to include all relevant details. We would recommend sending it to us before your first appointment, this will allow us to set up your electronic health record in advance, but it can also be completed when you arrive for your first appointment.
When joining Mungret Medical Centre you will be booked generally with either Dr Brian McEllistrem or Dr Muireann Clifford for the initial consultation. If you have a preference in terms of which doctor you see please let us know.
